WASHINGTON CAMPUS

Southmont—a trusted, person-centered care community in Washington, PA.

Voted No. 1 in Skilled Nursing Care!

You’ll find short- or long-term skilled and intermediate nursing care, rehabilitation services and hospice and palliative care at our Southmont skilled nursing community. Southmont offers secure, comfortable and engaging environments as well as services designed to best meet each resident’s needs.

You’ll have the peace of mind that comes from knowing your family member is in the experienced hands of Presbyterian SeniorCare Network, which has provided distinguished care and services for more than 85 years. It’s based on meeting each person’s needs and deinstitutionalizing the experiences so that older adults are able to live their lives to the fullest. We go above and beyond providing exceptional care on our Washington Campus! We enhance quality of life. As an example, Southmont hosts an annual Senior Prom, complete with fancy dress, music and all the excitement of socializing with neighbors and reminiscing about long-ago dances.

Rehabilitation & Skilled Nursing

Experienced social workers, registered nurses, licensed practical nurses, and certified nursing assistants (CNAs) work with each resident, the family and the physician to develop a specialized plan of care. At Presbyterian SeniorCare Network, we embrace a collaborative approach to maximize each patient’s physical, emotional and spiritual well-being. Our team is here to help with personalized rehabilitation options, including physical, occupational and speech therapy. Click here to learn more about our rehabilitation services.

Woodside Program

Our Woodside neighborhood at Southminster Place offers separate dining and special activities for individuals who are living with Alzheimer’s and dementia.
