Here you’ll find engaging daily programs and activities designed to meet your physical, emotional, social, spiritual and intellectual needs. Our caring staff will support you or your loved one in an atmosphere of dignity, independence and choice by offering assistance with medication monitoring, personal hygiene and grooming, dining services, housekeeping and laundry.

You’ll find a modern, home-like atmosphere that fosters a sense of comfort and security. Our expert Skilled Nursing teams include registered nurses, licensed practical nurses, certified nursing assistants (CNAs) and social workers who partner with you, your loved ones and our doctor to provide a personalized care plan.

For rehab, we help you get back on your feet and back to your life. With our MyLife program, your rehab is created to suit your desires and preferences. We strive to return you to your highest level of function to assure a quick, safe discharge from our care. Our team is here to help with award-winning rehabilitation options, including physical, occupational and speech therapy.

We offer specialized dementia support both at our Southminster Place personal care community as well as at our Southmont skilled nursing care community. Residents receive individualized care and engaging opportunities that create a quality of life not possible within a traditional dementia care setting. As your loved one’s needs change, our highly trained and skilled team adjusts care levels accordingly. We also offer caregiver-focused opportunities such as family support groups.

Woodside Place offers a holistic approach that meets the physical, social and psychological needs of persons living with Alzheimer’s disease and related dementia conditions. The Woodside model has been replicated nationally and internationally and remains the gold standard in dementia care. In addition to full-time residential living, Woodside Place offers full day and half-day adult day services.

Presbyterian SeniorCare Network and its affiliate, SeniorCare Network, in conjunction with the U.S. Department of Housing and Urban Development, have created Bellemead Apartments for seniors 62 and older and adults 18 years of age or older with mobility impairments. Residents of Bellmead Apartments must meet certain eligibility requirements and pay 30% of their adjusted gross income for rent.
