For our Residents…

Information about residents and/or members of the communities and programs of the Presbyterian SeniorCare Network is private information. The resident’s right to privacy is of utmost importance and is protected by Presbyterian SeniorCare Network policy and federal and state laws and regulations.

Upon the admission process to a health care center, each resident will be provided a Notice of Privacy Practices Notice. The Notice includes all elements and statements that are required by law. The Notice informs the resident of the uses and disclosures of Protected Health Information (PHI) that may be made by the community, the resident’s rights with respect to his/her PHI and the community’ legal duties with respect to such PHI. Please click here to view a copy of the Presbyterian SeniorCare Network Notice of Privacy Practices.

Additionally, to enhance coordination, Presbyterian SeniorCare Network is a member of ClinicalConnect HIE, western Pennsylvania’s first and largest exchange enabling the secure and timely sharing of digital healthcare records between participating hospitals and other providers. Please click here to view a copy of the Clinical Connect Health Information Exchange Standard Addendum to the Notice of Privacy Practices which is applicable to all of communities.

All information concerning donors or prospective donors, including names, the names of their beneficiaries, gift amounts and values of estate commitments will be held in strictest confidence and will not be shared with a third party.
